Fulham GP welcomes new patients. Please download this form and bring along with you to your first appointment or alternatively arrive 10 minutes prior to your appointment to complete a form.
Fulham GP provides a service where you may receive an SMS or letter reminding you of routine preventative health checkups, reviews or screening (such as pap smear tests or GP management plans).

Please make an appointment for repeat scripts if these are needed.
Request for Medical Certificates & Referrals
Please make an appointment for medical certificates or referrals. They cannot be backdated or provided for over the telephone.
Communication via electronic means (such as email) does carry privacy risks and other associated risks in that the information could be intercepted or read by someone other than the intended recipient. Written consent will need to be obtained from the patient prior to any personal or sensitive information being transmitted via email.

Our practice is committed to protecting our patients’ privacy according to the Privacy Act. The practice runs on a computerised, paperless system. Access to computer records containing your information is restricted to medical staff and password protected, with all hard copies of patient documents disposed of in a secure manner.
